On behalf of Avera Health, thank you for taking the time to assist your colleague by acting as a reference.

 

To begin, click on the Get Started button below and follow the instructions as they appear on the screen.

While completing the online questionnaire, you may view the applicant’s signed release at the top right corner of the screen.

After completion of the questionnaire, you will sign it electronically via DocuSign. IMPORTANT! Please be sure to complete the DocuSign process by clicking on the FINISH Button. If successfully completed, you will receive confirmation from DocuSign in the form of an emailed copy of your signed submission for your records. At that point you will be finished, and we will be able to access your response.
